Observation

110-300-0145(1): The outdoor play area had several garbage items scattered around the yard, including plastic juice packs, cans, and plastic cord-like strings. 110-300-0145(11): The gate was not self-closing or self-latching; the spring was detached. 110-300-0145(8): The emergency gate had a 4-inch gap underneath, and the white fence leading to the emergency gate had a 5-inch gap underneath one section. Additionally, the gate itself had a rotten wood panel on top that was exposing nails and splintering parts.
